Consent, Waiver & Appointment Policy Form

It is the intention of Golden Bond Jewelry, its employees, agents, and contractors (collectively referred to as the “Company”), to provide permanent jewelry services using precision welding equipment (“Services”). Before receiving Services, the person receiving the Services (“Releasor” or “I”) must read and sign this Consent & Waiver Form to acknowledge the potential risks associated with welding permanent jewelry, agree to the Company’s appointment and warranty policies, and release the Company from liability.

APPOINTMENT POLICY — “If You Give a Mouse a Cookie” (The No-Show Fee)

We’d love nothing more than to skip the credit card and operate on the honor system — but after one too many no-shows that affected more than the booking party (and others waiting to be serviced), we’ve learned that a little structure keeps the magic going for everyone.

A valid credit card is required to hold every appointment. I understand and agree that:

•  Any no-show, late cancellation, or no-call appointment cancelled within 24 hours of the scheduled start time will incur a $25 no-show fee, automatically charged to the card on file.

•  Cancellations or reschedules made more than 24 hours in advance will not be charged a fee.

•  The fee applies per scheduled guest on the appointment.

MINORS POLICY

Golden Bond Jewelry provides welding services to individuals aged 16 and 17 without the need for a parental consent form. However, for anyone under the age of 16, parental or legal guardian consent is required. This consent must be provided via a signed form at least 72 hours before the appointment. Parents or guardians must contact Golden Bond Jewelry directly to arrange and submit the necessary form in advance.

ACKNOWLEDGEMENT OF RISKS

I acknowledge that permanent jewelry involves welding jewelry onto my body using a welding machine (the “Activity”), which carries risks including, but not limited to, serious injury, pain, suffering, disability, death, property damage, and/or financial loss. I am aware that the welding process may involve sparks, minor electrical shock, or exposure to heated materials, leading to potential harm.

Additionally, permanent jewelry is designed to remain in place without removal, which may result in injury if the jewelry becomes caught or trapped. I am also aware of potential allergic reactions, infections, or discomfort from prolonged metal contact with my skin. I affirm that I do not have any known allergies, conditions, or disorders that could increase the likelihood of injury or complications from permanent jewelry.

By signing this form, I confirm that I fully understand and accept these risks and voluntarily choose to participate in receiving the Services.

WAIVER & RELEASE

By signing this Consent & Waiver Form, I hereby irrevocably waive, release, and discharge Golden Bond Jewelry, its officers, directors, employees, contractors, affiliates, and assigns (collectively, the “Releasees”), from any and all liability, claims, or causes of action that may arise out of or are in any way related to the Services, including but not limited to injury, illness, disability, death, property damage, or financial loss. This waiver applies even if caused by negligence on the part of the Releasees, except in cases of willful misconduct or gross negligence.

I agree that this waiver extends to any and all claims, whether arising under state, federal, or local law, including the laws of Virginia, Maryland, the District of Columbia, or any other jurisdiction where the Company operates. I further agree that any dispute arising under this agreement shall be governed by the laws of these jurisdictions.

I understand and agree that I am solely responsible for any medical treatment, transportation, or related costs incurred due to injury or illness arising from the Services. I acknowledge that any claims for damages shall be limited to the amount paid for the Services.

WARRANTY & NO REFUNDS

All sales are final. All permanent jewelry and pre-made jewelry from Golden Bond Jewelry are non-refundable. Permanent jewelry is custom-made and fitted for each individual, and once a chain has been cut and welded, it cannot be returned or refunded under any circumstances.

Complimentary Service Guarantee. Golden Bond Jewelry stands behind its work. We provide 

•  One (1) complimentary reweld, refitting, or repair on each permanent jewelry purchased.

•  This complimentary service covers issues such as the chain falling off, breaking at a weld point, or needing a minor sizing adjustment.

•  If the same piece of jewelry breaks a second time, Golden Bond Jewelry may, at its sole discretion, recommend a full replacement chain (purchased at standard pricing) rather than performing an additional repair. This is because repeated breakage typically indicates the chain style is not well suited to the wearer’s lifestyle.

Lifestyle Changes. If the permanent option no longer suits your lifestyle, we offer the option to add a clasp to your jewelry for a fee.

Refunds will not be offered after chain as been cut. Services have been initiated and rendered, regardless of changes in personal preference, lifestyle, sizing, or satisfaction with style or appearance after the welding is complete.

SEVERABILITY & GOVERNING LAW

This Consent & Waiver Form shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia, without regard to its conflict-of-law principles. If any provision of this Consent & Waiver Form is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, that provision shall be modified to the minimum extent necessary to make it enforceable, or, if it cannot be so modified, shall be severed from this agreement. The remaining provisions shall continue in full force and effect.

I confirm that I have read and understand the terms of this Consent & Waiver Form — including the Appointment Policy, No-Refund policy, and the Complimentary Service Guarantee — and agree to proceed with the Services.
